Please fill out the information requested
in the above spaces. An RMA number will be
issued after this information, along with a
hard copy of your purchase order is received
(via fax or mail) by Pulizzi Engineering,
Inc.
All RMA's must have a customer assigned PO
number. This PO shall authorize either: (A) $
90.00 per unit for no trouble found (N.T.F.)
on warranty units, or (B) Fifty (50) % of the
purchase price or $ 125.00 (which ever is
greater), for repair of non-warranty units.
If the repair cost turns out to be greater
than this estimate, customer will be notified
prior to repairing. Otherwise, customer will
be billed actual repair cost, not exceeding
(B) above.
Debit Memo's are not accepted under any
circumstances, all original invoices must be
paid within 30 days of shipment.
RMA numbers are good for 30 days only. If
shipment is not sent to Pulizzi Engineering,
Inc. within 30 days please call for a new RMA
number. *** Pulizzi Engineering's RMA number
must be written on the outside of the
shipping container or the unit will be
refused and sent back to the originator.***
All freight will be Prepaid and Billed unless
unit is in warranty. (Please see PEI
Terms and
Conditions)
Ship RMA units to: Pulizzi Engineering, Inc.,
3240 S. Susan Street, Santa Ana, CA
92704-6865. Units returned within 30 days of
purchase must be shipped in original
packaging.
Each box shipped must include the
RMA number on the shipping label. Pulizzi
Engineering will not be responsible for
damage that occurred during shipping.
